Thyroid radiofrequency ablation

What thyroid RFA is

Radiofrequency ablation uses a high-frequency alternating current delivered through a thin needle electrode placed inside a thyroid nodule. The current generates frictional heat in the tissue around the electrode tip, raising the temperature enough to cause localised coagulative necrosis. Over the following weeks and months the treated tissue is reabsorbed and the nodule shrinks, which can relieve pressure symptoms and visible swelling while preserving healthy thyroid tissue and function.

Because the treatment is delivered through a needle rather than an incision, thyroid RFA is typically performed as a day-case procedure under local anaesthesia, with no surgical scar and a short recovery.

How the procedure works

RFA of thyroid nodules relies on three techniques that the equipment is designed to support:

  • Ultrasound guidance. The operator watches the electrode in real time on ultrasound, both to position it accurately and to monitor the developing ablation zone as a hyperechoic change.
  • The moving-shot technique. Rather than holding the electrode in one spot, the operator divides the nodule into small conceptual units and moves the active tip from unit to unit, building up a complete ablation while staying clear of critical structures such as the trachea, oesophagus, and recurrent laryngeal nerve.
  • Internally cooled electrodes. The electrodes carry circulating coolant through the shaft, which keeps the tip from charring and lets the system deliver energy efficiently into the target tissue.

A trans-isthmic approach and continuous visualisation of the needle tip are standard practice to keep the procedure controlled and to protect surrounding anatomy.

Who it is for

Thyroid RFA is performed by endocrine surgeons, interventional radiologists, and endocrinologists trained in ultrasound-guided intervention. It is generally considered for patients with biopsy-confirmed benign nodules that cause pressure or cosmetic symptoms, and for selected autonomously functioning nodules, when the patient prefers an alternative to surgery. Patient selection, biopsy confirmation, and suitability are clinical decisions made by the treating physician.

Frequently asked questions

Is thyroid RFA a replacement for surgery?

It is an alternative for selected benign nodules, not a universal replacement. It aims to reduce nodule volume and symptoms while preserving thyroid function. Whether it is appropriate for a given patient is a clinical decision based on biopsy results and the individual case.

What consumables does each treatment use?
